The case in brief:

Jeremy was initially planned for a two-stage treatment:

  • A Mk1 immediate denture following extractions, retaining the upper canines after root canal treatment

  • A Mk2 metal-based denture nine months later

However, things didn’t go quite to plan…

After fitting the Mk1, Jeremy experienced:

  • Lisping on ‘s’ sounds

  • A feeling of the denture being too bulky, restricting his tongue

Despite multiple refinements, speech problems persisted. So we revised the plan:

  • Removed the canine roots

  • Made a Mk2 temporary complete denture to increase tongue space

  • Followed later by the Mk3 definitive complete upper denture, with a fully embedded metal base

The Mk3 design allowed me to adjust the polished surface precisely for speech, without any join between acrylic and metal that could be felt by the tongue. We used universal powder spray to assess and adjust for sibilance.
